Almost reaching his forties, plastic surgeon Sean McNamara struggles with the problems that is gradually tearing his so-called perfect life apart. After making peace with Julia, Sean and Julia get back together and have three children, one of which is physically disabled.

Sean manages the McNamara/Troy plastic surgery practice with Christian Troy, his best friend. It will be later on revealed that Christian is the father of Sean’s son, Matt. Sean’s second child’s name is Conor, whom he had just after he and Julia reunited.
